Step 1: Complete Our Online Intake Form (or Contact Us Directly)
Your journey begins by either filling out our online intake form or contacting us directly—whichever feels more comfortable for you.
Our intake form is a quick and easy way to see if you meet the basic qualifications to become a surrogate. It helps us learn more about your background and how we can best support you.

Step 2: Connect With Us Personally for an Initial Consultation
After you complete the intake form—or reach out directly—one of our experienced Intake Managers will contact you to schedule an initial consultation by phone call, text message, or email, depending on your preference.
This consultation is highly recommended, even if you’ve already submitted your form. It gives us a chance to introduce ourselves and learn more about you—your background, your family and support system, your surrogacy goals, and why you're considering this journey.
During this first conversation, we will:
- Listen to your story and understand what’s important to you
- Answer your questions about the process, timeline, and what to expect
- Explain the next steps, including documents, screenings, and matching
- Support you in deciding whether surrogacy is the right fit for you and your family
This step helps us build a strong foundation together. Our Intake Managers are here to support you with compassion, knowledge, and clear communication from the very beginning.
Step 3: Complete the Full Application
Once we’ve connected, we’ll invite you to complete our comprehensive online application and sign a few release forms. This step gathers important details about your pregnancy history, lifestyle, and preferences and helps us create your personalized surrogacy profile.
Step 4: Medical Record Review
After receiving your signed medical release form, our team will request your OB and delivery records directly from your healthcare providers.
Please note that we do not accept previously collected records from surrogate applicants, with the exception of individual test results such as a recent Pap smear or similar. We prefer to request the records ourselves to ensure everything is complete and nothing is missing.
Our experienced team will thoroughly review your medical history to confirm that you meet the health criteria required for surrogacy. These records are kept strictly confidential. If needed later in the process, they may be submitted—with your permission—to the intended parents’ fertility clinic for final approval.
Since collecting and reviewing records can take 1–3 weeks, depending on how quickly your providers respond, we greatly appreciate your honesty and transparency from the beginning. If you’re aware of any prior medical conditions or pregnancy complications, please let us know early. This helps us avoid unnecessary delays and ensures we don’t keep you waiting if there are strong disqualifying factors.

Step 5: Background Check & Household Verification
If your medical records are approved by GSHC, we’ll begin the remaining steps in your screening process. As you near the end, we’ll guide you through a few final verifications:
- A background check
- ID verification
- Submission of basic financial documents (e.g., a paystub or tax return)
- A Zoom call to confirm your identity and living environment
- A partner interview, if applicable
These checks help us ensure a stable, supportive foundation for your surrogacy journey. You’ll be guided every step of the way by a caring, knowledgeable team.
Step 6: Final Consultation, Profile Completion & Agreements
Once you’re medically and personally approved, we’ll schedule a final consultation to prepare you for the next stage. During this meeting, we’ll guide you through completing your surrogacy profile—a thoughtful, anonymous introduction that intended parents will review when considering a match.
We’ll also walk you through the key agency agreements, which outline expectations, responsibilities, and your official readiness to move forward. This consultation is an important opportunity to ask questions, clarify anything you're unsure about, and ensure everything feels aligned before proceeding.
Our team will take the time to explain every detail so you feel fully informed and confident before signing. Once your profile is finalized and agreements are completed, you’ll officially be ready for matching.
Step 7: Matching with Intended Parents
With your profile and agreements complete, you’ll officially begin the matching process.
We carefully consider your preferences and values to find intended parents who are the right fit. When a potential match is found, we’ll share their profile with you and arrange a virtual introduction. Matching is always a two-way process—both you and the intended parents must feel comfortable and aligned.
Once everyone agrees, the match becomes official, and you’ll move on to legal contracts and clinic screenings.
