Donor-Conceived Person Information Requests

GSHC Surrogacy Agency helps donor-conceived persons and parents or legal guardians of minor donor-conceived persons request information from records maintained by our agency.

Who may request information?

  • Donor-conceived persons aged 18 or older: May request donor-identifying information and non-identifying donor medical history, where disclosure is required by applicable law.
  • Parents or legal guardians of a minor donor-conceived person: May request non-identifying donor medical history. This does not provide the same right to donor-identifying information as an adult donor-conceived person.

The information available for release depends on applicable law and the records associated with the donation. Contact us if you are unsure whether your request is covered.

To begin, please provide:

  1. Your full legal name.
  2. Your contact details and preferred way for us to respond.
  3. Whether you are an adult donor-conceived person or the parent or legal guardian of a minor donor-conceived person.
  4. A brief description of the information you are requesting.

Identity and eligibility verification

Before information is released, we need to verify your identity and your eligibility to receive the requested records.

  • Adult donor-conceived persons: Government-issued photo ID, such as a driver's license or passport, to verify identity and age. We may also ask for information reasonably available to you to connect your request with the relevant donor records.
  • Parents or legal guardians: Government-issued photo ID and documentation establishing your relationship or legal authority to act for the minor donor-conceived person.

Please contact us for secure document-submission instructions before sending copies of identification, guardianship documents, or other sensitive records. Do not attach these documents to your initial email.
